I use and really like FormSpring (<-- Disclosure: my affiliate link).

Wufoo is another good one, but I haven't used it that much. Both offer a free plan; they have paid plans if/when you need more features.

There are a lot of free ones for wordpress at wordpress.org that works fine.

Wufoo is really good IMO...I was using EmailmeForm or something like that and tried Wufoo and immediately swapped all my stuff.

At the moment I'm using the java script, because when I change something in the form, it automatically updates it, so I don't need to go back and re-copy-paste the html in my files.

User-end wise though, html loads pretty fast, while there's sometimes a 1 -2 sec. lag with the java script.

Yeah, I found out about Wufoo b4 FormSpring, but at the time Wufoo didn't allow multi-page forms and some other advanced features. But I see now that they offer all that. So either one in my opinion is a good choice.
