Good solution for a virtual terminal / accept credit card payments via phone?

I need this for my main business.
I know there are lots of solutions out there. Anyone has experience with this?
here's the main criteria for me:

• low fees (of course)
• all payments in Euro on my side, so I need a solution where payments go through in Euro, no money losses because of exchange rates
• reliable interface
• reliable payments
• low fees for money payouts to my german bank
• no hassle with having to send them loads of company documentation, bank referrals
