Form generation software

I wasn't sure where to post this, so if Mods feel this is in the wrong area then please feel free to move it.

Basically, I'm looking for some kind of software that will allow me to generate forms on A4 paper. Trying to set up cells and checkboxes in Word is a complete pain and there must be a more user friendly piece of software that will allow me to generate a well designed form that can be sent via snail mail.

Anyone know anything that they can recommend?
